Job Position's Description
Registered Nurse Primary Function:
- To provide and ensure quality and safe delivery of nursing services.
- To coordinate provision of nursing services to assigned Patients that reflects the Agency mission and standards.

Essential Duties and Responsibilities:
- Initial and ongoing comprehensive assessment of the client’s needs, including Outcome and Assessment Information set (OASIS) assessment at appropriate time points.
- Initiating the plan of care and revising as necessary.
- Providing those services and/or treatment requiring substantial and specialized nursing skill.
- Counseling the client and family in meeting nursing and related needs.
- Initiating appropriate preventive and rehabilitative nursing procedures.
- Preparing clinical and progress notes. Evaluating outcomes of care. Planning for the discharge from services.
- Coordination of services and referral of other services as needed.
- Maintain communication with physicians, family members and other agency personnel to adequately plan for and provide the patients’ needs.
- Supervising LPN/LVN’s and paraprofessionals providing services.
- Attend and participates in continuing education programs.
- Pull, push and or lift 15-20 pounds on a daily basis. Pull / push 125-200 pounds on a regular basis. Pulls/push 200 or more pounds on a regular basis. Is subject to falls, cuts bruises, aggressive patients, odors, chemicals, etc. throughout the work day. - Provide periodic progress records and report to assigned patient accounting to current policies and procedures.

Essential Duties and Responsibilities:
Clinician paperwork: Develop, maintain, and enforce accurate systems for submitting clinician paperwork/telephony calls, reconcile all clinician paperwork, verify paperwork received is complete and accurate and redistribute for corrections as necessary, develop and maintain accurate tracking systems for 5-day turnarounds and recertification, maintain systems to track daily patient notes, develop and maintain systems for notifying clinicians of paperwork due to complete patient release.

Certified Nursing Assistant Primary Function:
Under the supervision and guidance of a licensed nurse, the CNA is responsible for assisting with direct patient care within the scope of their practice as well as other work on the unit which supports the patient environment. The CNA assists staff to ensure optimal patient care and assists the healthcare team to provide and maintain a clean, safe, and attractive environment for patients. Work will include components of direct patient care, nutrition, observation, documentation, transportation of patients and supplies, and general maintenance of the patient environment. The CNA is responsible for maintaining his/her own clinical and mandatory education requirements and for meeting the established standards of their profession as well as those accountable for his/her practice, assigned duties/patient care, and is an advocate for patients/families and their profession.
